This course provides practical ways to identify a patient’s caregiver team and tools for documenting caregiving teams. This course can be taken on its own or as a supplement to the foundational course. 

Objectives:

  1. Name key roles and responsibilities of caregivers.
  2. Describe how and when to identify members of a patient's caregiver team.
  3. Recognize options for documenting caregiver teams.
