Join Anna Chodos, MD, MPH, Executive Director of Dementia Care Aware; Barbra McLendon, MSW, Associate Vice President of Public Policy; and Jennifer Schlesinger, MPH, CHES, VP of Healthcare Services & Professional Training at Alzheimer’s Los Angeles for a discussion on Dual-Eligible Special Needs Plans.

 

This webinar will provide a high-level overview of D-SNPs and DHCS expectations and clarify where dementia screening and care fit within those requirements. Speakers will explore how to operationalize meaningful dementia care through navigation and specialist support, and review how Dementia Care Aware can serve as a practical strategy and scalable model for implementing dementia screening within D-SNP networks.
