This course is designed to help hospital teams identify, engage, and support caregivers in hospital care and discharge planning.

Objectives:

  1. Describe the benefits of caregiver engagement for hospital care and transitions.
  2. Describe how to identify members of a patient's caregiving team and include them in early care planning.
  3. Demonstrate how to include caregivers as an integral part of your discharge planning and patient's care.
