Join Dementia Care Aware, LifeLong Medical Care, and the Institute for Healthcare Improvement (IHI) to explore how age-friendly care principles translate into ambulatory clinical settings. The session focuses on mentation, the 4Ms domain most closely tied to dementia screening, and how health systems can go beyond Age-Friendly recognition to implement it meaningfully. LifeLong Medical Care shares their experience as an Age-Friendly certified FQHC and how partnering with Dementia Care Aware helped them deepen their mentation practice.
The webinar will be held on Zoom through this platform. Participants of the live webinar are eligible to receive 1 CE/CME credit, including AMA, ANCC, AAPA, and ASWB credit.
